Basic Instructions and Form Downloads
- Unless notified otherwise, the quarterly evangelism report is no longer required. This page is retained in case it is reinstated.
- Download the form here for pdf, here for Microsoft Word, or here for Microsoft Excel.
- Always complete the last column PLUS all quarters that have passed this year. Beginning with the second quarter, each quarterly report you send expands on and may correct your previous one, so show the ENTIRE year to date so that we may replace your previous quarterly report with the new one.
- See "Guidelines Regarding the Supplement Section of the Report" (below).
- Fax to (850) 434-0119 PLUS e-mail if possible.
- If you don't have a fax and can't e-mail, be sure to send via U.S. Mail in plenty of time for it to be received by the deadline. Mail to the District Administrative Assistant at The Pensacola District Office, P.O. Box 2727, Pensacola, FL 32513-2727.
Guidelines for the 4th Quarter's Supplement Section of the Report
- Only the final quarterly report for the year (due in January) will require a Supplement section.
- The Supplement reports, by category, the date, name, gender, and race for all those added and subtracted from the professing membership roll for the entire year. (Examples of "category" are as follows: added by profession of faith, transferred to/from another UMC, removed by charge conference action, deaths, etc.)
- If a person transferred to/from a church, show the name of the church.
- There is no official Supplement form - but do NOT send a print-out straight from your membership program!!
- Please properly prepare your report. Save paper. Be concise. If your Supplement is multi-page, fill each page as much as possible. If you must use an auto-print-out from your membership program, cut-&-paste sections to shorten it.
Confirm That the District Office Received Your Report
For even the most prompt and efficient, sometimes e-mails don't go through, mail is slow or misdirected, and faxes fail. This section is about procedures to follow to confirm whether your report reached the District Office.
- If receipt is not confirmed below within 2 days of when we should have received it, please resend it. If you sent it by e-mail the first time, please fax it (if you have a fax machine) to (850) 434-0119.
- If receipt is still not shown within one day, please call the Pensacola District Office at (850) 434-0118.
- If a mailed report is addressed to the DS it will be opened only by the DS, so there will be a delay in acknowledging receipt.
